Employer Forms
Download and complete our new employer intake form for streamlined billing. Employees bring authorization form to their appointment for streamlined processing.
Employee Authorization for Examination or Treatment
Employees who need exams or treatment should bring their completed "Authorization for Treatment" form to their appointment for streamlined processing.
Other Forms
Download and complete any pertinent form prior to your visit. This will expedite your care at the clinic.
